Veterinary Resources

When local veterinary practitioners volunteer to assist during a disaster, they are not always able to work from their practice or have access to a medical record system. During a disaster, documentation of the veterinary care provided is more important than ever as owners may not be able to be reached, or animals have been impacted or injured during the incident. To support this effort, CVET has developed species specific exam forms to assist in the documentation of veterinary exams, treatments, recommendations, owner communications, etc. Local animal responders providing veterinary care are encouraged to use these forms to provide a template of consistent documentation practices during disaster operations.

In addition, a form to record daily veterinary requests is also included below to allow animal caretakers, shelter workers/volunteers, and CART members to identify concerns and request a veterinary evaluation of animals under their care and provides a workflow for the local veterinarian to follow.
